Appointment scheduling

Online requests typically receive a response within 1-2 business days. If you are trying to schedule a same-day or next-day appointment, please call.

Appointment cancellations
Please call at least 24 hours before your scheduled appointment to cancel so that your slot can be given to another who needs it.
No-show policy
A no-show fee will be applied if the office is not notified 24 hours prior to a missed appointment. This is done to ensure patients that we have plenty of time to spend with you and your family.
What to bring
Please bring your insurance card or information and any referral documentation, as well as any prior imaging, work-up, or any relevant health records.
